Nurse led Continuum of care approach for addressing diabetes in Nepal

Dhulikhel Hospital in close collaboration with World Diabetes Foundation implemented a project “Nurse led Continuum of care approach for addressing diabetes in Nepal” which went from Feb 2018 to November 2021. This project involved the nurses from the clinical and community department to take an Dhulikhel Hospital in close collaboration with World Diabetes Foundation implemented a project “Nurse led Continuum of care approach for addressing diabetes in Nepal” which went from Feb 2018 to November 2021. This project involved the nurses from the clinical and community department to take an active role in screening from the community to diagnosis and linkage of the patient as a continuum of care. With the intention of sustainability, the diabetes patient recognized at the community level are still in contact for the fasting and postprandial blood glucose monitoring and contact with consultants and the medicine refill of those patients still going on after the completion of the project.
