The department of Internal medicine was established since the time the hospital had started its clinical services in 1996. The department constitutes one of the major departments in the hospital in terms of number of outpatient and inpatient service it caters to the patient population. In addition, it has also been producing internal medicine specialists since many years and sub-speciality in cardiology since recently. It works in line with the principles of Dhulikhel hospital; and aims to provide quality health care to all ! The department always ensures that medical knowledge is up-to-date ; focuses on regular upgrading; and the service delivery is cost-effective and affordable. The department has nine faculty members with seven residents along with medical officers and specialized nursing staffs.
The department of internal medicine has the largest number of outpatient visits in the hospital amounting to more than eight thousand patients per week .It has been providing regular outpatient services through its general OPD clinic, cardiology clinic, gastroenterology clinic, pulmonology and endocrine clinic from 8:30 AM to 4 PM, six days a week. .
The department of internal medicine provides 24 hour emergency service via its on-floor and on-call specialist manpower. It also provides emergency endoscopic and PCI services to those who warrant treatment .
The department has 50 inpatient beds equipped with continuous ECG monitoring and resuscitation unit. It has modern ICU/CCU unit which is taken care by holistic team of specialized physicians, anesthesiologists and trained nursing staffs.
Apart from its medical services, department of internal medicine is regularly providing patient education regarding chronic diseases like COPD, diabetes mellitus, hypertension etc. Through these programs patients are educated about their disease conditions, possible complications, drugs and insulin administration techniques, dietary habits and medical nutritional therapy.
Hospital has one of the best cardiology unit with the leading edge technologies with well-equipped cath lab and modern infrastructure which provides regular angiography and angioplasty services. We have dedicated CCU provides 24 hours cardiac emergency services. We provide services like tread mill test, echocardiography, ambulatory BP monitoring, Holter monitoring. Apart from the services in hospital, the department is actively participating in programs creating public awareness regarding cardiac diseases and screening school children for rheumatic heart diseases. A special focus is given on prevention of cardiac diseases in which patients are counseled for a healthy heart and handling emergencies.
The department has two interventional gastroenterologists providing uninterrupted services throughout weekdays. Biliary and pancreatic interventions via high-end endoscopic services like ERCP /EUS has established the unit as the highest volume referral center in Nepal. The gastroenterology unit performs more than thousand ERCPs per year along with several other therapeutic endoscopic treatments receiving patients from all districts of Nepal.The department is also equipped with technologies like Argon laser (APC) , Spyglass DS , RFA for biliary tumors, EUS ( FNA/EUS-guided Cystogastrostomy/Necrosectomy) and provides regular and emergency endoscopic interventional services like foreign body removal, variceal band ligation, endoscopic hemostasis (clipping, sclerotherapy, Glue injection) and SEMS insertion etc.
Pulmonology unit is functioning as an integral division under Department of Internal Medicine. It is primarily looking after various lung diseases such as Chronic Obstructive Pulmonary Disease, Bronchial Asthma, Interstitial Lung Disease, Lung cancer, pleural diseases, chest infections including pulmonary tuberculosis, etc. Moreover, it is also looking after post covid patients with persistent respiratory illness every week. Besides regular in-and out-patient services under this division, it has two spirometry machines performing pulmonary function test and DLco. Diagnostic bronchoscopy is being done regularly. In near future, it aims to extend its services in interventional bronchoscopy, medical thoracoscopy, pleural and lung biopsies etc.
The department has been providing hemodialysis services for chronic renal failure patients as well as acute renal failure patients. Emergency hemodialysis facility has also been provided for ventilated patients in ICU.
The department provides endocrinology services especially through diabetic and thyroid clinics . One of the senior faculty is currently doing specialist fellowship in Endocrinology.
The department has been providing antiretroviral therapy for the HIV/AIDS patients through well trained physician and nurses. It also provides comprehensive care for the people living with HIV.
The department is also looking after neurological disorders and started acute thrombolysis service in acute stroke in cert with neurosurgeon of the hospital.
The department is actively involved in teaching MBBS, Bsc. Nursing, Bachelor in Nursing Sciences, Bachelor in Dental Surgery, Bachelor in Physiotherapy and Bachelor in Pharmacology students. The department has its own post-graduate course in MD internal medicine and DM cardiology.
